Are you a licensed healthcare professional looking for opportunities beyond the traditional clinical setting? (i.e. Nurse’s Aide, Medical Assistant, LPN or Registered Nurse)
Become a Community Health Aide Worker and manage patients remotely!
- Monitor and work with patients virtually
- Help chronically ill patients remain proactive to avoid clinical episodes
- Assist patients with self-reporting vitals
- Transport patients to medical appointments when necessary
- Identify and help patients reach health goals
- Review and update patient care plans
- Perform monthly virtual wellness screenings
- Serve as a liaison between patient, family members and the patient’s physician

As a companion driver you play a critical role in keeping our aging population connected to a sense of community and helping them avoid isolation.

As a Companion Driver you can also work with clients by providing companion care, helping them complete household chores, providing personal shopping & delivery services when clients are unable to leave their home.

Qualifications:
Community Health Aide Worker
Companion Driver
- Well maintained personal 4 door vehicle/clean driving record
- Previous experience in senior care or other related fields
- Healthcare certification – (i.e. CNA, LPN, R.N. etc.)
- CPA/ First Aide Certified
- Compassionate and caring demeanor
- Clear driving record and criminal background
- Professional References
- Ability to build rapport with clients
- Coachable, go-getter, entrepreneurial minded, self-starter and team player.
- Must be experienced in updated technology for patient monitoring systems and comfortable using mobile app and telehealth portal
- Access to mobile device for work (i.e. cell phone, tablet etc.)
- Willing to travel to all major metro hubs in Virginia (Charlottesville, Richmond, Virginia Beach, Norfolk etc.)
